Planning ahead

We are proud to share with everyone our success for the participant Bob and his loved ones since we have stepped with our coordination of support.

We came into contact with Bob and his family earlier this year in February.  Bob is living with younger-onset dementia which has been impacting on his cognitive and social function.

As Bob’s growing concern requiring close supervision due to his absconding incidents occurred recently. The family was also experiencing other hardships including his ageing parent’s ailing health and the constant worry of a longer-term solution to provide the appropriate level of care as Bob’s condition progresses.

It was clear that Bob needed increased assistance due to a change in his complex care needs and planning ahead.

Since our intervention, we discussed the NDIS housing options.  Bob and his mum & dad want him to move out of his parents’ home in a planned basis, it would prevent out of home care in response to crisis, if, his ageing parents are unable to continue to care for him.

Two weeks ago, Bob’s new plan with an appropriate funds become available.  It allows Bob take steps to explore his long-term housing options.

We will continue our efforts in extending our support for the family and keep striving toward positive outcomes.
